This page contains lists of descriptions and links for FORMS available to both members and visitors of St. Matthew’s church. Each item contains a brief description of the form and a link to the form or PDF document.

  • Room Requests
    • All room requests should be submitted using thess forms
      • Requests from internal St. Matthew’s groups
      • Requests from external groups
  • Check Reimbursement Form
    • Supports individuals purchasing supplies/food/etc for ministry events

  • Subscribe to Weekly Word
    • Weekly Word is St. Matthew’s newsletter delivered by email each Friday.

  • Subscribe to Children and Family announcements
    • Announcements focused on Children and Families generated when necessary.

  • Submit Weekly Word Announcement
    • Please allow at least two weeks before event
    • Non-event announcements should be submitted by Wednesday each week

  • Submit Website Update Request
    • Please allow 2-3 weeks for routine website updates
    • Major website changes or requests for new pages will be considered on a case-by-case basis and may require additional time
